Program Costs
Registration fee: $35 per clinic
Current PAYING 5th Grade Extended Care Students - $195 ($65/month)
Non-Extended Care 5th Grade Students (sibling dismissal = non-paying) - $255 ($85/month)
6-8 Students - $195 ($65/month)

*Funds are non-refundable for absences; withdrawal refunds prorated at month's end for remaining months only.

By typing your name below you agree to the following:

• I understand my child will be enrolled in the 5-8 CoEd Basketball Clinic above.
• Upon enrollment my FACTS account will be immediately charged for the non-refundable program registration fee(s).  Depending on the option I selected, my FACTS account will be invoiced for program fee(s) on 2/1 for the pay-in-full option, or 2/1, 3/1 & 4/1 for the monthly option.  I also understand I will not be reimbursed for any of my child's absences or emergency school closures, and if my child withdraws, it will be effective month-end, and future monthly invoices will be canceled.

By typing your name below you agree to the following:

• I authorize my son to tryout for the 5th-8th Grade Boys Flag Football Team on Fri. 2/2/24 from 3:50-5:20pm.
• I understand there are 12 spots available on the team, and if my son is selected I am responsible to pay the $35 registration fee and the following program fee: 
  6-8 grade students: $195 ($65/mo) 
  5th grade PAYING Extended Care students: $195 ($65/mo)
  5th grade non-Extended Care students: $255 ($85/mo)
• I will take him for a sports physical at his primary care physician
• I understand my son's presence at the Sunlight Boy's Flag Football games is important and agree that he will be present at the games, to the best of our ability and in the exception of illness.
• Upon acceptance to the team my FACTS account will be immediately charged for the non-refundable program registration fee(s).  Depending on the option I selected, my FACTS account will be invoiced for program fee(s) on 2/1 for the pay-in-full option, or 2/1, 3/1 & 4/1 for the monthly option.  I also understand I will not be reimbursed for any of my child's absences or emergency school closures, and if my child withdraws, it will be effective month-end, and future monthly invoices will be canceled.

• I authorize my daughter to tryout for the 5th-8th Grade Girls Cheer Team on Tues. 1/30/24 from 3:50-4:50pm.
• I understand there are 12 spots available on the team, and if my daughter is selected I am responsible to pay the $35 registration fee and the following program fee: 
  6-8 grade students: $195 ($65/mo) 
  5th grade PAYING Extended Care students: $195 ($65/mo)
  5th grade non-Extended Care students: $255 ($85/mo)
• I will take her for a sports physical at her primary care physician
• I will purchase bloomers & socks.
• I understand my daughter's presence at the Sunlight Boy's Flag Football games is important and agree that she will be present at the games, to the best of our ability and in the exception of illness.
